Our professional staff tailor care to meet individual needs while providing an encouraging and comfortable environment for addressing acute medical conditions and/or issues related to aging.

What We Offer

Transitional Care
Augustana of Minneapolis houses two transitional care neighborhoods within our care center. Private and semi-private rooms are available to individuals requiring rehabilitation services, medical monitoring, short-term respite, and/or other transitional care services. Each neighborhood contains a spacious lounge area for dining, social events, and gathering with friends or family.

Dementia Care
Dementia-related care is a hallmark of the care center as we cater to individuals at all stages of impairment. Our social model centers around the belief that persons with dementia, even at the latest stage, continue to live meaningfully. We do this by focusing on capabilities rather than limitations. Our caring staff and volunteers lead an Alzheimer’s support group available to the families and friends of individuals living at Augustana.

Rehabilitation Services
Onsite rehabilitation services include physical, occupational, and speech therapy and are provided by Augustana Therapy Services. Through state-of-the-art methods and technology staff aim to stabilize health conditions, promote wellness, and enhance comfort of life.

We offer ecumenical worship services in our chapel. Our chaplains provide spiritual care to residents and their families. The Health Care Center is part of the larger Augustana Minneapolis campus including senior living apartments, medical services, and many amenities easily accessible to those living within our community.
