The basic definition of a skilled nursing facility (SNF) is:
A state licensed institutional setting which provides nursing and rehabilitative care provided by or under the direction of skilled medical personnel – available 24-hours a day & ordered by a physician under a treatment plan.
Augustana Health Care Centers provide care for people whose medical needs require the attention of licensed nurses, but not the more intensive care of a hospital. Social workers and case managers help seniors and their families with insurance issues and the coordination of nursing care plans. Dietitians, physical and occupational therapists and other health professionals help support and sustain seniors’ physical and emotional well-being.
Augustana is recognized as a premier service provider for seniors, 55 and older, in the Twin Cities and outlying areas. We have over 1,200 employees and own and manage over 800 nursing home beds and 1,100 senior housing units.
