Augustana’s Transitional Care Units (TCU)
Augustana’s Transitional Care is the nurturing “in between” care environment for individuals who may not require a hospital stay, but need a place where they can progress in their healing. Augustana Transitional Care provides you with the therapeutic and rehabilitation services you need and comfortable, private rooms to give you personal space.

When moving from one level of care to the next, our registered nurses, rehabilitation therapists and other staff help you to reach your goals and improve your health so your transition can be smooth and effective. Upon admission, our team will work with you and your family to determine your objectives and therapeutic plan in order to help you achieve and maintain restored health. We offer all therapy and medical technology services on-site and 24-hour registered nursing means that quality care is always available.
